For your health and safety, the Texas Department of Licensing and Regulation (TDLR) requires that we have a New Client Intake Form for every new client on file, and we abide by all regulations.
Our Fillable Client Intake Form for Therapeutic Massage can be downloaded below and completed before you visit our office. Please bring it with you when you arrive for your appointment.
And please arrive 15 minutes before your appointment time, because our Massage Therapists must review your New Client Intake Form and complete a Massage Consultation Form, which you must also sign, before your services can begin. We are required to keep both on file.
